What is Pet Insure?

Pet Insure is an insurance designed for the furbabies and their pawrents. Pet Insure offers a 3-in-1 package benefit such as Medical Reimbursement, Pet Owner’s Liability, and 24/7 pet owner’s personal accident cover.

What dogs can be covered?

Pet Insure is insuring home pets or companion dogs regardless of the breed from six (6) months up to eight (8) years old.

Pet Insure does not cover the following:

    • Stunt pets
    • Used for military, police or security services,
    • Raised and used for fighting,
    • Raised and used for sports
    • Less than six (6) months nor more than eight (8) years old.

Deductibles
Section 1 – Medical Reimbursement PHP 5,000.00 each and every loss

Euthanasia Burial Assistance Benefit – NIL

Section 2 – Owner’s Liability Third Party Bodily Injury – NIL

Third Party Property Damage – 5% of loss amount, minimum of PHP 5,000.00, each and every loss

Section 3 – Personal Accident Cover/td> NIL

Who can avail this product?

You can avail of Pet Insurance if you:

    • are within 18-65 years old
    • are a Filipino or legal Philippine resident
    • have a GCash account
    • have a pet dog raised for companionship purposes only between the age of 6 months and 8 years

How do I pay for my insurance?

Premium payment will be deducted from your GCash wallet

What shall be done after purchase?

You will need to activate your policy within 7 days. Otherwise, your policy will not be activated. Payment will be refunded to the GCash wallet.

When will my coverage start?

Your coverage begins at 12:01 AM Philippine Standard Time the day following the date of activation.

List of illnesses covered:

The policy does not specify which illnesses are covered. Exclusions include:

  • Pre-existing conditions
  • Inherited/congenital conditions
  • Related to breeding, pregnancy, whelping, nursing
  • Bilateral conditions (conditions pre-existing on one side of dog prior to policy inception)
  • Related to anal glands, behavior, teeth
  • More than one illness/injury per coverage period due to repetitive, specific, similar activity
  • Internal/external parasites
  • Illnesses for which vaccine is available
  • Obesity or overweight, if not due to an underlying medical condition
  • Osteoarthritis, spondylosis, luxating patella and diabetes

    • General Requirements

      1. Certificate of Cover and Copy of Policy, or Policy Number
      2. Government issued I.D./Company or School I.D. showing complete home address and signature of Insured, with photo
      3. Pedigree Certificate and/or RFID or microchip scanned serial number, only if submitted during policy issuance
      4. Pet Photo
      5. Written Incident Report

      In addition to the general requirements, the following documents must be submitted:

      A. Section 1 Medical

      Reimbursement For Sickness/Illness Claims:

      1. Pet Baby Book/Veterinarian’s Records, and Certification from  the  attending Veterinarian on the illness or nature of treatment undertaken, and health condition of the pet
      2. Prescription of the Veterinarian for Medical Treatment
      3. Original Receipts of Medical Treatment and Medication of the pet

      For Accident-Related Claims

      1. Pet Baby Book/Veterinarian’s Records, and Certification from  the  attending Veterinarian on the illness or nature of treatment undertaken, and health condition of the pet
      2. Prescription of the Veterinarian for Medical Treatment
      3. Original Receipts of Medical Treatment on the pet
      4. Police Report or Affidavit of how the accident happened

      For Burial Assistance Benefits Claims (For Euthanasia only)

      1. Pet Baby Book/Veterinarian’s Records, and Certification from  the  attending Veterinarian on the illness or nature of treatment undertaken, and health condition of the pet
      2. Veterinarian’s Certification and reason for euthanizing the pet.

      B. Section 2 – Owner’s Liability

      For Third Party Property Damage Claims

      1. Written demand of the third party
      2. Itemized list of damaged property/ies
      3. Photographs of damages items/property
      4. Estimate of repair/replacement of damaged item/invoice/quotation
      5. Quit claim of third party

      For Third Party Bodily Injury Claims

      1. Written demand of the third party
      2. Death Certificate (Original/PSA Copy) of third party (in case of death)
      3. Medical Certificate/Clinical/Emergency room report of third party’s bodily injury/ies
      4. Original official receipt/s of medical expenses incurred by third party
      5. Statement of account from the hospital (if confined)
      6. Employment certificate with monthly compensation (if any)
      7. Valid ID of third party
      8. Quit claim of third party

      C. Section 3 Personal Accident

      For Accidental Dismemberment and/or Disablement Claims

      1. Certified true copy of Certificate of Confinement or Medical Certificate
      2. Statement of account / hospital billing and records of confinement (original copy)
      3. Operating room report (if an operation was done)
      4. Certification from the attending physician if total disability
      5. Picture of claimant with the affected part

      For Accidental Death Claims

      1. Valid I.D. of Insured pet owner and his/her beneficiary
      2. PSA Death Certificate (original and authenticated)
      3. Affidavit of how accident happened or duly notarized police report
      4. Certified true copy of Post mortem exam if claimant died in the hospital
      5. PSA Marriage Certificate (original and authenticated) – if beneficiary is spouse
      6. PSA Birth Certificate of claimant (original and authenticated) – if beneficiary is parent/child
      7. PSA Birth Certificate of claimant and beneficiary (original and authenticated) – if beneficiary is brother/sister
      8. Filled out Client Information Sheet of beneficiary
  • How are the benefits paid out? How much can I get from my Pet Insurance?
    • The claim proceeds are paid through Fund Transfer. We will be requiring the following documents:

      • Certification bearing bank account details (preferably RCBC account; other bank accounts will still be subject for management’s approval) duly signed by the Insured. (Template will be provided)
      • Valid ID of the signatory on the release documents
      • Any document that will prove or show Insured’s bank details
      • Signed Release Document (Template will be provided)
  • How much can I get from my Pet Insurance?
    • This is subject to the submitted official receipts for reimbursement and the limit of coverage. Items excluded on the policy will not be reimbursed.

Cancellation

Can I cancel my policy?

Yes, the insured may opt to cancel the policy and may be entitled to a premium refund subject to the short-period rate scale. In this case, please contact Malayan Insurance.
